LONG TERM CARE


accommodation and is regulated by the Ministry of Health and Long-Term Care. Additionally, the Home has one short stay bed for respite purposes and 3 dedicated veteran’s beds. All admissions to the home are through the Hamilton, Niagara, Haldimand, Brant Community Access Centre with eligibility according to the MOHLTC requirements. The philosophy of resident care is based on an individualized care plan that is developed in conjunction with the resident and their family. Lead by the Home’s Medical Director, an interdisciplinary team provides 24 hour care and services to the residents.


John Noble Day & Stay Centre: Located within the John Noble Home, the Day & Stay Centre offers specialized programs for persons living in the community that are diagnosed with Alzheimer’s or a related dementia. Services provided include, days for individuals with memory problems who need motivation and stimulation, an early onset/early stage program for persons starting out on the dementia journey and overnight respite to assist with caregiver relief. All programs are designed to promote independence and have a wellness focus, provided within a secure home-like setting. The Day Program operates Monday to Friday and includes door to door transportation within Brant County. The Overnight Program is offered every weekend, starting Friday evening and runs through to Sunday afternoon. In partnership with the Alzheimer’s Society of Brant, the Day & Stay Centre offers an early onset/early stage program with transportation on Thursday afternoons which includes a dementia support group where individuals can share their feelings as well as participate in specialized recreation programs and have the opportunity for socialization. All programs offered by the Day & Stay Centre can be accessed through the Hamilton, Niagara, Haldimand, Brant Community Access Centre.


BELL LANE TERRACE


Bell Lane Terrace is a 26 unit affordable housing apartment complex geared to seniors. The apartment complex contains a bachelor, and one and two bedroom apartments with some of the units modified for complete accessibility.

Priority is given to spouses of residents residing in the John Noble Home. The apartment units are spacious with a common room and laundry room located on each floor. A beautifully landscaped garden is attached to the building. The apartment complex is located adjacent to the John Noble Home and has a separate entrance and parking lot. Access to these apartments is through Brant Housing.

Leisureworld Caregiving Centres operate 19 provincially regulated long-term care facilities in Ontario, including the one in Brantford. The Brantford facility has 122 beds and two respite beds. The facility has bingo, social teas, and spa days and prides itself on its menu. The Centre is spacious with several rooms available for events and activities. Park Lane Terrace is an accredited long–term care facility located in the north end of Paris on 30 acres of landscaped grounds. A walkway surrounds the building and there is an enclosed garden for the safety of those who may wander. The interior of the residence is spacious and features fireside lounges and many other areas for quiet conversation or relaxation. Park Lane strives to meet the physical, emotional, social, educational, psychological and spiritual needs of each resident. Four self-contained, furnished resident home areas offer basic, semi-private and private rooms with ensuite washrooms. Couples suites are available to meet the needs
