to Mr. John Henshaw as he departs his post as Executive Director of the Maine Port Authority. John has been a guiding light to the Port Safety Forum during the past 10 years. His leadership and friendship are valued and we wish him the very best in his future endeavors. His vision, passion and drive have opened new trade opportunities in Maine emphasizing the State’s value as a transportation hub. The Port Safety Forum also says good
bye to LT David Bourbeau of Sector North- ern New England as he moves on to a new role at the Coast Guard Academy and to LCDR Meghan McGovern of NOAA, who has been an outstanding advocate for navi- gation safety. She leaves us to move on to an Executive Offi cer position aboard a NOAA research ship. The Port Safety Forum welcomes Mr.
Matt Burns of Maine DOT. Matt is a Maine Maritime Academy graduate and is working exclusively with the Maine Port Authority. Matt will be taking the interim helm of the Maine Port Authority. The Forum also welcomes LT Matt
Odom, who took over the Waterway Man- agement Division at Sector Northern New England. LT David Vejar of NOAA, also joined the Port Safety Forum. He assumed the role of LCDR McGovern and is based in Rhode Island. Introductions and Administration: Introductions – The meeting was called
to order at 10:00 by the Port Safety Forum Co-Chair Matt Burns representing the Maine Port Authority with Commander An- drew Meyers as the Co-Chair representing the Coast Guard. A quorum was comprised of 26 individuals. Review and Approval of the June 14, 2017 Port Safety Forum Meeting Minutes – Hearing no objections to the content of the June 14, 2017 minutes, Mr. Burns accepted the minutes as approved. Two amendments were made to the agenda. Mr. Kelliher was unable to attend the meeting due to an emergency, thus the Right Whale Topic under Item 2 Port Activities was deferred. Mr. Carroll Brown added Private Aids to Navigation to the agenda under Item 2 Port Activities.
Port Activities Update: Pilot Boarding Incident – Brian Downey; Maine Pilotage Commission Mr. Downey shared the fi ndings of a re-
cent pilot incident which he investigated on behalf of the Maine Pilotage Commission. On July 1, 2017 at approximately 0932, a pilot fell from a pilot ladder while boarding a U.S. naval vessel enroute to Eastport. During the boarding evolution, the pilot boat deployed the pilot on the pilot ladder on the ship’s starboard side. The ladder was in “like new” condition. The ship’s fl are extended nearly the entire length of the ship including the pilot boarding station, which caused the pilot ladder to hang freely down without contacting the side of the ship. At the point of embarkation, the pilot ladder was approximately two feet from the hull. The pilot climbed onto the ladder when
the pilot’s left leg cramped. The pilot boat captain recognized that the pilot was having trouble on the ladder and quickly brought

the boat back to the pilot ladder to attempt to recover the pilot. Although, hanging to the ladder with his arms through the rungs (at shoulder/head height) he ultimately could not sustain his grip and fell off of the ladder and into the water. Once in the water, the pilot’s infl atable Type V lifejacket auto- matically deployed upon being submerged, as designed. The pilot was recovered by the pilot boat crew within approximately 2 minutes. The water temperature was 48°F. All safety gear worked as designed and
proved instrumental in the safe recovery of the pilot. The crews of both the U.S. naval vessel and the pilot boat are to be commend- ed as their expert prowess in emergency procedures assured a near textbook man overboard recovery.
Sarah Long Bridge Replacement Update – Ben Walz, MaineDOT Mr. Ben Walz outlined the current sta-
tus of the Sarah Long Bridge replacement project. The project, which is led by Maine DOT is moving along well. The replace- ment includes signifi cant construction in and around the channel in the vicinity of the existing bridge. Since the last MaineDOT
update to the Port Safety Forum in Septem- ber of 2016 Maine DOT has accomplished the following milestones: Completed the tower footings; Completed the demolition of the Old Bridge (some dredging remains); Completed the approach piers & abutments; Completed the casting & erecting of all 88 tower segments; Completed the casting of all of the vehicular and rail road segments, and erected all but 18 vehicular segments; and Assembled the lift span A channel closure will be required
between October 17-27. MaineDOT is partnering with all concerned parties includ- ing, the Coast Guard, pilots, and waterway users to ensure that traffi c can be diverted or re-scheduled to avoid channel confl icts. During the closure, the lift span will be fl oated into place and installed (on October 18). Other upcoming bridge work includes: Complete the erecting of the vehicular seg- ments (early October); Complete placing vehicular and rail road curbs; Paving the bridge deck; and Demobilize of access tres- tle.
