Patient Instructions*: Present this card and your insurance card along with a valid prescription for EXFORGE® (amlodipine and valsartan) tablets at any participating pharmacy or through mail order. You will be responsible for the first $4 of your co-pay and the card pays up to the next $50 for 30 tablets. Offer expires 12/31/2013. Patient questions should be directed to: 1-877-699-9973.
Pharmacist instructions for a patient with an Eligible Third Party: Submit the claim to the primary Third Party Payer first, then submit the balance due to Therapy First Plus as a Secondary Payer as a co-pay only billing using a valid Other Coverage Code, (e.g. 8). The patient is responsible for the first $4 and the card pays up to the next $50 for 30 tablets. Reimbursement will be received from Therapy First Plus.
Valid Other Coverage Code required: For any questions regarding Therapy First Plus online processing, please call the Help Desk at 1‐800‐422‐5604.
*Valid for those with private insurance. Not valid under Medicare, Medicaid or any other federal or state program, for residents of MA or cash-paying patients, or where product is not covered by patient’s primary insurance. Limitations apply. This card is the property of Novartis Pharmaceuticals Corporation and must be returned upon request. Novartis reserves the right to rescind, revoke or amend the program without notice.
