ONE OF THE HIGHLIGHTS OF WORKING ON SUCH A LARGE SCALE EU-WIDE PROJECT HAS BEEN THE OPPORTUNITY TO COLLABORATE WITH COLLEAGUES FROM MANY COUNTRIES, EACH WITH THEIR OWN AREA OF EXPERTISE


The programme officially began in November 2019 and will run for 48 months until October 2023.


The SHAPES innovation action aims to create an open European digital platform that facilitates the provision of meaningful, holistic support to older people living independently.


Pertinent to the topic of digital solutions for medicines management is the ‘Medicines control and optimisation’ pilot theme, whereby the SHAPES platform and selected digital solutions will be used to monitor health condition(s), physiological parameters and medicine adherence.


Within this project are seven pilot themes investigating various potential uses of the platform: smart living environment for healthy ageing at home; improving in-home and community-based care; medicines control and optimisation; psycho-social and cognitive stimulation promoting wellbeing; caring for older


individuals with neurodegenerative diseases; physical rehabilitation at home; and cross-border health data exchange.


‘Medicines control and optimisation’ - one of the seven themes being explored in the campaign - will investigate the impact of digital solutions that aim to optimise medicines use by way of fostering effective self-management, while facilitating timely intervention by clinicians-based on remote monitoring and individualised risk assessments powered by artificial intelligence.


MOIC is leading the medicines control and optimisation pilot, which will focus on identifying, managing and improving deficiencies in adherence to medicines and treatments of older individuals living with permanent or temporary reduced functions or capabilities due to chronic, age-related illnesses and living at home.


The extensive pilot plan – which has been created for all pilot sites to follow – involves a clinician monitoring a participant’s medicine control, vital signs and health and wellbeing,


receiving notifications about his or her risk of deterioration via clinician-facing dashboard and responding appropriately.


Digital Solutions (including blood pressure monitors, pulse oximeters, weight scales and glucometers) will be used to monitor the individual’s health conditions, physiological parameters.


Participants in this pilot theme will be individuals who are over 65 years old, live at home and have multiple chronic medical health issues - specifically cardiac, respiratory and endocrine conditions.


If successful, the SHAPES Innovation Action will lead to a greater sense of self-sufficiency and empowerment in people living with chronic conditions as they grow older.

Through MOIC’s participation in projects like SHAPES we are able to provide the people of Northern Ireland with access to cutting-edge healthcare technology with the aim to support people to living independently for longer.


‘A key challenge of this project is to make the technology as accessible as possible, to allow as many people take advantage of the many benefits we hope it will bring.


‘The project is funded for four years ending in November 2023 when we hope to have the full evaluation available.’
