Complete Immunization Form


Immunizations are an important part of college community health. Not only do they protect you, but they also protect your fellow students, family, faculty and staff. All on-campus students must satisfy all immunization requirements by the first day of classes or a non-refundable $50 Non-Compliance Fee will be applied to your student account. To avoid this fee:


1. Create a Patient Portal account at patient-winthrop.medicatconnect.com.


2. Complete the required forms, enter your immunization dates, and upload your immunization documentation (Paper forms will NOT be accepted).


Winthrop University Health Services can provide anything you need to meet your immunization requirement for a nominal fee.
