This knowledge is then cascaded throughout a hospital department; people don’t become ‘task-focused’ and less latent errors are introduced,” Gale continues. He explains that legislation, governance and the variety of safety nets that are designed to mitigate against error are written and established with the expectation that everyone will do what they are supposed to do.
