This knowledge is then cascaded throughout a hospital department; people don’t become ‘task-focused’ and less latent errors are introduced,” Gale continues. He explains that legislation, governance and the variety of safety nets that are designed to mitigate against error are written and established with the expectation that everyone will do what they are supposed to do. However, when we have a sub-culture or a ‘culture within a culture’, we can build in and normalise unsafe practice. People can circumnavigate the safety nets that are put in place to protect patients and staff. “This is why it is vital to explain why we do things a certain way – if you tell someone the reason why we are doing something, then people invest in it; if they invest in it, it becomes a part of them,” Gale asserts.
