How Air Abrasion Has Revolutionised The Way I Treat Fissure Caries In Children

“Here we go again!” I thought, as yet another gadget appeared in my surgery.
“I think you’re going to like this” my principal exclaimed. “It multi tasks beautifully and is easy to switch from air polishing and desensitising to air abrasion for simple cavity preparation.”
Sceptical as I am, I smiled sweetly and promised to give it a go.
My referral; a fourteen year old female needle-phobic, had recently been assessed using the ICDAS with a score of two requiring biopsy of early fissure caries. My patient required investigation of very early fissure caries in three of her permanent first molars. Following discussion with her dentist and her Mum about treatment options she agreed to undergo treatment using the Velopex Aquacut with fine grade aluminium oxide air abrasion.
After explaining what she should expect to feel - slight tingling from the powder, gritty sensation and the possibility of cold sensation on the tooth - we agreed that, if it was not suitable, conventional drill based treatment could still be used along with local anaesthetic if required. Treatment commenced and my patient reported slight sensitivity during preparation but not enough to warrant anaesthetic as the sensation subsided as soon as the Aquacut ceased.
Following treatment I asked her for her honest feedback, and she confirmed slight sensitivity, not warranting local anaesthetic. She liked the fact that all three teeth had been treated in a short space of time with relative ease and no drill noise or vibration. She would definitely have treatment with the Aquacut again.
