Tomosynthesis Improves Care and Reduces Recalls Tjongerschans Hospital, Heerenveen, Netherlands

Tjongerschans Hospital added Hologic’s tomosynthesis technol- ogy to its breast imaging deport- ment and the staff and patients are noticing a difference. “I am convinced I have seen more

cancers with tomosynthesis,” states Radi- ologist A.G. Janssen, MD. “We also have reduced the number of women we recall for additional tests.”

Located in the Friesland district in the north Netherlands, Tjongerschans Hospital is the first hospital in the region to offer the new breast imaging technology. “We have been using tomosynthesis for only a few months, but the news has spread and women are coming to the hospital for our tomosynthesis imaging capability,” says Dr. Janssen. The hospital transitioned from analog to full-field digital mammography six years ago, installing a Hologic Selenia digital mammog- raphy system. In 2009, the hospital replaced the system with a Selenia Dimensions and upgraded this unit with tomosynthesis function- ality in 2011, enabling the hospital to take both 2D and tomosynthesis images with the same system.

Tomosynthesis

Increases the Accuracy of Digital Imaging

“Six years ago we saw a major advance in breast cancer detection with the addition of digital imaging,” states Dr. Janssen. “The technology provided a dramatic improvement in the quality of the images. Now tomo- synthesis’

‘three-dimensional’ digital

imaging makes digital mammography even more accurate. It makes me feel very comfortable making a decision about doing a biopsy or not. Tomosyn- thesis is a wonderful tool.” “With

screening, we take images

The hospital uses the breast tomo- synthesis system for diagnostic exams for select patients, including patients referred from the screening program, patients with palpable masses, pros- thesis, dense breasts, a family history of breast cancer, and prior breast can- cer.

“Tomosynthesis also can be very helpful in differentiating between a scar and cancer regrowth after surgi- cal removal of the cancer,” states Dr. Janssen. “We use tomosynthesis for problem solving in general.”

Dr Janssen says

“....I am very enthusiastic about tomosynthesis. It makes my job easier. I’m much more confident

when I make a decision...”

With the Hologic tomosynthesis sys- tem an x-ray tube rotates in a small arc across the breast taking 15 images in a single compression. The system reconstructs the breast in a series of 1 mm slices and radiologists can exam- ine the images individually or in cine mode, assisting in the identification of abnormalities hidden by dense or over- lapping tissue. Dr. Janssen was familiar with the

traditional two-dimensional two

of each breast and sometimes we wish we could see more,” says Dr. Janssen. “With tomosynthesis can see more.”

Breast tomosynthesis eliminates the problem of overlapping tissue or structural noise.

Hologic technology through his work as a radiologist with the Netherlands Breast Cancer Screening Program. The program offers free screening mammograms every two years to women between the ages of 50-75. “I read more than 20,000 mammo- grams annually,” states Dr. Janssen. “The national screening program uses Hologic Selenia systems in stationary and mobile units, so I was very familiar with Hologic. I have been interested in tomosynthesis from the start and decided to add the technology after reviewing the research data.”

The Technology is Easy-to-Implement and Reduces Patient’ Anxiety

According to Dr. Janssen it was not difficult to add tomosynthesis to the workflow.

“It is not time consuming

for the technologist or the radiologist and the images are not difficult to inter- pret. Everyone is happy with it,” states Dr. Janssen. “Tomosynthesis also saves patients a lot of anxiety,” explains Dr. Janssen. “When we tell a woman we see some- thing and she needs additional imag- ing or a biopsy, she is in shock. Tomo- synthesis is very important because it reduces the number of women we send for additional tests.” “I am very enthusiastic about tomo- synthesis,” concludes Dr. Janssen.

“It

makes my job easier. I’m much more confident when I make a decision.”
