Texas physicians who provide immu- nizations should perform at least one test by submitting patient immunization data to ImmTrac, the state’s immuniza- tion registry, www.dshs.state.tx.us/im munize/immtrac/imm_providers.shtm. Physicians who do not provide im- munizations should choose the public health measure on syndromic surveil- lance data submission. In Texas, only the Tarrant County Public Health Depart- ment can electronically receive syndrom- ic surveillance data. Physicians who do not submit to Tarrant County or who do not collect syndromic surveillance data can claim an exemption. Physicians participating in meaning-

exclusion from both public health mea- sures, he or she should select one of the two measures during attestation and claim the respective exemption. He or she should then choose four other mea- sures from the menu set for attestation. Physicians who need help with at-
