Rejuvenation of the hand

Uwe Wollina presents the findings of a small, uncontrolled study into the effects that peels and dermal fillers can have on the rejuvenation of the hands

ABSTRACT

Signs of the ageing hand in Caucasians, including pigmentary changes, fine wrinkling and actinic keratoses, have been addressed with serial treatment of topical 10% capryloyl salicylic acid (CSA) peels. In this pilot study, 10 adult Caucasian patients were investigated to determine the effects of CSA peels on skin rejuvenation. CSA was applied every other week for a series

of four to six treatments. The effect was photo-documented, and a patient’s and physician’s global assessment performed. For the assessment of hand ageing, the validated hand grading scale was also used. Improvements following treatment were further validated using the Williams clinical hand assessment. Clinical assessment using these different tools demonstrated an improvement in all cases.

he ageing hand has become a matter of increasing interest in aesthetic medicine. Thinning of the skin, loss of volume, pigmentation spots, deformities of joints and nails, wrinkling and prominent veins are regarded as the main

The procedure itself has only mild temporary adverse effects, such as erythema, stinging sensations, and scaling; while the skin’s texture improves, and pigmentation and keratoses can be reduced significantly. Skin radiance also improves. With complementary sun protection, the effects of treatment will remain for at least 24 months in the majority of patients. CSA peels can also be combined with other procedures, such as dermal fillers.

removal of pigmentary spots and hyperkeratoses since the 1980s and 1990s, respectively. Topical drugs for depigmentation and the removal of hyperkeratotic lesions, like actinic keratoses, are often used as a first-line treatment. Dermal fillers and autologous fat grafts are also used for volume restoration4, 5
