authorities. They are now working together to deliver the most challenging of ambitions. It is what might be described as a ‘big ask’. Collaboration, at the best of times, takes enormous personal effort and understanding, which takes time to develop. This has all to be up and running in order to deliver its critical services, which are already under stress. Evidence suggests that integrated care may improve patient satisfaction, which currently is at an all time low, as well as improve access to services and perceived quality of care. However, evidence of effect on resource use and health outcomes is limited – potential benefits may be modest and take time to be realised.6
Conclusion The many factors which contribute towards the acute challenges noted above are predicated on a number of different elements. Firstly, the workforce situation will vary from one ICS to another, with shortages being most likely in London where living is so costly. Other areas where shortages are most acute are rural England, where housing is scarce and transport less and less reliable. In addition, the geography and population size of the ICS makes a huge difference, especially if there are historical governance and financial difficulties between the parties.
ICSs with a past of working together and in a small geographical will be able to establish themselves more quickly than large complex areas with varied partnerships, which will take longer to settle into their new roles. One part of the ICSs’ important tasks is to address the
shortage of GPs, given the numbers of elderly and frail with complex needs. Shortages in general practice are not evenly distributed and highlight an issue that will potentially cause great challenges, among many others. Lack of certainty with the social care system and local council financial strains are also a cause for specific concern. However, that topic deserves its own focus and article.
