Under the general direction of the Supervisor of CaseManagement, these professionals will play an important role in the collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy for options and services to meet the comprehensive health needs of each patient and/or family.Utilizing effective communication skills and available resources, these individuals will promote quality, cost-effective outcomes, including care coordination for complex and catastrophic conditions, and support Hospital Transition of Care and PCMH programs.


Requirements include: RN licensure –Maryland, 4+ years of RN experience in Acute care (Adult ComplexMedical/Trauma/Critical Care) including case management experience in Home Health, Hospice or Hospital CaseManagement. Must be able to conduct acute evaluations telephonically and performthe discharge planning of patients fromhospital to home or a facility. Must be willing to obtain paid certification in CaseManagement within 18 months of employment.
