MTI doctors have to grapple with an unfamiliar clinical context and patients with needs and perceptions that they have not been exposed to before.


However, it is not just the MTI doctors’ clinical ability that has a positive impact. Dr Ruparelia has been struck over the years by the strong work ethic of doctors on the scheme, who demonstrate an enormous amount of respect for their UK colleagues and gratitude for the opportunity to train alongside them. They see the unit almost like a family, wanting to support their team in any way they can. Dr Ruparelia recalled one doctor who insisted on coming in to support colleagues on his day off. He could not stand the thought of something bad happening and not being there to help his team. This has built strong relationships between the MTI doctors and the staff at the Whittington and they maintain lasting friendships long after they have finished their placement.


The experience of an MTI doctor From 2013 to 2015, Dr Mohammed Rishard, from Sri Lanka, was an MTI doctor in post at the hospital. Many Sri Lankan doctors have joined the MTI scheme over the years, passing their MRCOG exam and using their experience to develop their skills even further on their return to Sri Lanka. Like his colleagues, Dr Rishard excelled on the MTI scheme, passing his MRCOG and completing two Advanced Training Skills Modules (ATSMs) in the short time he was in the UK. He also contributed a massive amount to his department by teaching junior doctors and being involved in undergraduate education. He found the two years a hugely rewarding experience: “The working environment at the Whittington Hospital helped me to get through the Part 2 MRCOG in a shorter

period of time than I expected. This achievement is a great milestone in my career. “Apart from the training in general in


O&G, I have successfully completed ATSMs in medical education and hysteroscopic surgery. This exposure has definitely helped to boost my confidence and skills. “I also enjoyed the ATSM in medical education very much. The curriculum covers a broad spectrum and this training has helped me to understand the concepts of the tasks I had to perform very well.” Dr Ruparelia and her colleagues aim to


support each MTI doctor to leave the UK with a new skill – whatever is achievable and useful for their future careers. This ranges from supporting a Singaporean doctor to have CTG training, who then went on to introduce a new CTG service in Singapore, to getting them involved in Practical Obstetric Multi-Professional Training (PROMPT) and Managing Obstetric Emergencies and Trauma (MOET). All of these enhance the MTI doctors’ professional development and often these skills are transferred overseas as they take on teaching roles on returning home. Dr Ruparelia would strongly encourage other hospitals to provide placements to MTI doctors. MTI doctors are reliable, want to work and learn, and go out of their way to get the most out of working in the UK. Dr Ruparelia has been struck by the dedication expressed by MTI doctors to use the opportunity provided by the scheme to improve women’s health care in their own countries, and she recalled what one MTI doctor saw as his duty to his country: “I owe my country for training me and I can’t go back empty-handed.”

Rishard MRCOG Dr Rishard graduated from The Faculty of Medicine, Ragama, Sri Lanka. He then


underwent postgraduate training in obstetrics and gynaecology in various teaching hospitals in Sri Lanka and obtained his MD from the Postgraduate Institute of Medicine, Colombo in 2012. He was the coordinator of the Academic Activities Committee of the Sri Lanka College of Obstetricians and Gynaecologists in 2012 and was awarded the ‘Young Gynaecologist Award’ by the Asia & Oceania Federation of Obstetrics & Gynaecology in 2013. Dr Rishard joined the RCOG’s MTI programme in 2013, becoming a Member of the College during his placement in 2014.
