*If you choose to pay by check, an invoice will be emailed to you. Registration is not complete until fully paid. You can then submit to your finance department for processing.

All cancellation requests must be received in writing via fax or email prior to June 4, 2012. Non-attendance does not constitute a refund request.


An 80% refund will be given if request is received prior to May 4, 2012.


After May 4, 2012, all requests will only be considered in the event of extenuating circumstances.

Please note that no cancellations of membership will be accepted in the case of conference cancellation.


Program Cancellation Policy: In the unlikely event of conference cancellation, APIC will refund 100% of registration fees paid. APIC assumes no liability for any penalty fees on airfare tickets, deposits for hotel accommodations or any other fees, charges, penalties, or other incidental costs that a registrant might incur as a consequence of cancellation.

Contact Hours Information The Association for Professionals in Infection Control and Epidemiology (APIC) is approved for providing continuing nursing education by the following organizations:


ANCC Continuing Nursing Education Contact Hours APIC is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation (ANCC).


One contact hour = 60 minutes. California Board of Nursing


APIC is approved for providing continuing nursing education by the California Board of Nursing, provider number CEP 7146.


Continuing Medical Laboratory Education (CMLE) APIC 2012 is recognized by the American Society for Clinical Pathology as meeting the criteria for CMLE credit. ASCP CMLE credit hours are acceptable to meet the continuing education requirement for the ASCP Board of Registry Certification Maintenance Program.
