Wound Treatment with LIGASANO® for Ulcer on the Achilles Heel


Practical experience report by: Dr. med. Michael Jecht, physician at Havelhöhe Hospital and Astrid Kliem and Susanne Hangen, nurse therapists for wounds in Berlin, Germany


Patient details and medical history: 71-year-old patient, diagnosed with type II diabetes, in- sulin-dependent, diabetic neuropathy, stroke in 2014, right hemiparesis, wheelchair-bound; very good home situation. Wound description: Ulcer over the right Achilles heel since 10/2015, probably due to kicking against the wheelchair. Treatment from 10/2015 to 11/2015: hydrogel; gauze with vaseline (Lo- matüll), 12/2015: start with medical honey and Silvercell, 01/2016: back to hydrogel from Hartmann, 02/2016: con- tinued with Silvercell (granulation at the wound edge)


03/2016: change to foam with adhesive edge (Aquacel Foam). Condition of the wound on 06.04.2016: wound fissured, inflamed, with fibrin coatings, partial granula- tion; slight wound odour; wound area: approximately 12 cm long, 5 cm wide, 0.5 cm deep, skin around the wound dry and scaly, minimal pain but sensitive to touch. The patient is switched to LIGASANO®; ulcer has remained unchanged under the previous treatment; wound size and signs of inflammation have tended to increase. Wound cleansing with Prontosan wound irrigation solu- tion, wound bed Prontosan Woundgel X, dressing mate- rial see below.


6th April


Condition of the wound before treat- ment with LIGASANO® white


20th April


Size of the wound now 11x5x0.5 cm, fewer coatings, otherwise unchanged


10th May


The wound looks significantly better, with further reduction of coatings


1st June


The depth of the wound has decreased and shows several granulation islands
