For post-transplant heart recipients
being treated at Papworth, dd-cfDNA testing of all cfDNA samples using the Alloseq NGS kits will now take place and clinical service using the PAXgene tubes and Maxwell RSC for DNA extraction is planned from September 2026. The research team is also looking
at post-transplant paediatric kidney recipients with a research study to evaluate dd-cfDNA in a paediatric kidney population, using PAXgene tubes and Maxwell RSC for DNA extraction.
Conclusions As experience grows across UK transplant centres, dd-cfDNA is steadily moving beyond the research seting and into
routine clinical evaluation. Collaborations between clinicians, clinical scientists and researchers are helping to build the evidence and practical experience needed for wider adoption. While dd-cfDNA has significant potential to improve post- transplant surveillance, its successful adoption will depend on robust laboratory workflows, standardised methodologies and continued clinical validation. The value of bringing together
researchers, clinicians and laboratory scientists at events such as Maxwell in Focus lies not simply in sharing new data, but in exchanging the practical lessons that will help accelerate the safe and effective implementation of emerging technologies. As dd-cfDNA continues
its journey from research into routine practice, these collaborative discussions will play an important role in shaping the future of minimally invasive transplant monitoring in the years ahead.
