CARE PLANNING
Collaborating for improved outcomes
Care providers and suppliers have one common goal: ensuring the people accessing care have the best possible outcomes. Sam Hussain, Founder and CEO of the care management platform Log my Care, delves into the importance of working together to improve social care.
As the care sector faces increasing pressure, keeping the people we support
at the core of what we do is more important than ever. This goes beyond delivering care that addresses a person's physical needs and focuses on their overall wellbeing. Ensuring they receive support tailored to their needs.
Care management platforms, like Log my Care, help care providers gain oversight of their service, streamline processes, define desired results for service users and set specific and measurable actions that increase independence. But we can only achieve this with input from the people we support and the numerous organisations working to improve the sector.
TALK TO CARE PROVIDERS AND THE
PEOPLE THEY SUPPORT The people our care management platform supports have always been at the heart of what we do. We designed our platform in direct response to people's needs. And every new feature we launch has been asked for, vetted and approved by care providers.
“Care teams have endless to-
do lists, but understanding their needs can lighten that load.”
For example, our customers told us they wanted a tool that would allow them to define desired outcomes and measure progress in a way that was meaningful to each person's individual needs. We heard again and again that there wasn't an existing solution for efficiently managing outcomes and goals.
In response, we built a fully flexible and functional goal management feature that works seamlessly with care planning and delivery – moving care providers away from outdated Word documents and making it simple to define goals and use photos and videos to track and evidence progress, easily demonstrating goal attainment to local authorities and families.

With our customer advisory board and community of care providers, we keep the conversation two-way. It’s easy to get caught up designing what you think is the perfect solution for care providers, but if you don't talk to them, it'll never be entirely right.

COLLABORATE WITH INDUSTRY EXPERTS
We work with passionate social care experts like Mark Topps and Adam Purnell. Ensuring we're up to date on hot topics and can lend our voice to the issues that need our support. We also use their expertise to create resources simplifying new guidance in the sector.

By collaborating with people who are actively improving social care and have experience working in care settings, we know our platform isn't just putting a bandage on social care. It's contributing to a better future.
WORK WITH STANDARDS BODIES
https://theprsb.org/
Our work with the Professional Records Standards Body (PRSB) ensures we take a unified approach to developing high-quality care records. The About Me standard puts the individual back into the heart of care. Ensuring the important details services users want to share with both health and social care are readily available.
By working together, we can take a strategic approach to the future of social care. Ensuring quality, efficiency, and innovation remain at the forefront of care delivery.
SUPPORT CARE ASSOCIATIONS
We're nothing without the wonderful care associations we partner with. They keep us on the pulse of care providers' needs by letting us know what their members require to deliver better care. With decades of experience in the sector, care associations provide a solid sounding board for our new features. Speaking to their members also helps us get valuable feedback on what needs to be fixed from providers who use other care management platforms. Knowing what's not serving social care is as helpful as knowing what is when building an easy-to-use and intuitive care management platform.
