says of the fiscal side to commissioning. It’s therefore no surprise that when he first qualified as a GP he got involved with setting up the practice-based commissioning group in the locality. He became a member of the professional executive committee (PEC) and chair for the last year before the reforms, putting him in very good stead to head up the new incarnation under clinically-led commissioning.
BETTER TOGETHER
The collaboration between Chorley and South Ribble and Greater Preston CCGs began then. “In the early days of our shadow period, or the early start of the reform, we had four PBC localities,” explains Dr Bangi. “What we did is look at who we would partner with in the future.” As a result, Chorley and South Ribble merged into one CCG, while the GPs in Greater Preston created a separate CCG. They started out by giving GPs a choice, as Dr Bangi explains: “We asked: ‘Do the GPs want one big CCG?’ But they didn’t. When we assessed our health economy, though, we decided very early on that, even with two CCGs, the only real way we could add value here was to work together.” Dr Bangi describes the collaboration further: “I’d say that clinically what the GPs have done here is say: ‘How does it work best clinically?’ When you look at the commissioning agenda and
the future requirements and issues around commissioning and then look historically at what the issues were, then actually the best way to do this was to run this economy around a fixed area – so central Lancashire – and our providers nicely fitted into one health economy. In many cases it doesn’t make any sense for us to work as two CCGs, because one day I could be sorting out some care pathways for Chorley and South Ribble CCG and then the next day it’d be Greater Preston CCG with the same community providers;
“We work together in such a close fashion that we’re essentially working as one. But there’s a lot of power in having two CCGs”
that would just lead to a lot of chaos. What we decided – and historically we’re used to working together on lots of things – was that we would just work together and bring major players in the economy to work with us on that philosophy.”
