www.nagor.com in the 1980s, idealised an implant to be
placed within the gluteal fat. Unfortunately this was accompanied by complications, until the procedure was corrected by Argentine surgeon Jose Robles. At the same time, the fat from
liposuction was used by many Latin American surgeons to remodel the buttocks, as well as increase volume and fill the lateral depressions. These procedures quickly developed
and influenced many performing submuscular buttock implantations. The procedure involves an incision of
up to 3 inches in-between the midline of the buttocks, within which a pocket is created in the soft tissue of muscle and fat.
Other implants Of course, now that the range of implants
available is vastly increasing, patients can choose implants to augment their biceps, calves and even pectoral muscles. Pectoral muscles are usually solid
silicone, inserted either through the armpit, lower breast area or areola. As with all implants, a pocket is made, this time under the muscle or under the connective tissue that covers the muscle. For calf implants, an incision
is made behind the knee and a pocket created. For biceps and triceps, and into the top of the armpit.
Lipofilling Fat transfer (lipofilling) for augmentation (particularly to the breast) has been used with liposuction for many years. However,

this has generally been criticised for various reasons, including fat dying and leaving cysts in the breast.. Thankfully, the techniques used have
evolved and companies such as Cytori Therapeutics have been able to develop excellent technologies and processes for these treatment indications. Aesthetic indications for
undergoing lipofilling include the disappearance of fat from the cheekbones, nasolabial folds, and in some instances of lines between the lower eyelids and the cheek (and not forgetting breast augmentation). Lipofilling can
also be used to smooth out irregularities such as those resulting from poorly performed liposuction. The necessary fat is obtained by a
limited liposculpture through one or more 3–5 mm incisions, usually taken from the abdomen or inner thigh. The aspirated fat is processed by centrifuging, filtering or rinsing. Pure liquid fatty tissue ready for injection is the result. The fat is then injected where needed
and evenly distributed by injecting minute amounts so that the injected fat is well surrounded by healthy tissue. This ensures that the transplanted fat remains in contact with the surrounding tissues, which must supply it with oxygen and nutrients.
