national bodies should share information and intelligence on the quality of services in an open and transparent way, and take coordinated action where appropriate in the event of an actual or potential quality failure.


Health and care professionals All staff working in health or social care have a responsibility to regularly review that the care they are delivering is of appropriate quality and meets the expectations of the public whom they serve. They should, on a regular basis be


analysing the data which is collected, via clinical governance and audit mechanisms; patient satisfaction surveys pertinent to their area; to measure themselves using the essential standards of quality and safety as well as national clinical guidance to ensure that they meet expectations. The drivers to improve quality and


safety are already in place in many instances, but the ‘first lines of defence’ rarely have, or take, time to review the available feedback and seek to make improvements in a systematic way using the data available. Root cause analysis tools are readily


available and need to be used, not only when events have caused harm, but also


as a proactive tool by the multidisciplinary team to review all the care they deliver. Regular improvements to practice must become much more of a way of life in the future and it is to be hoped that the new structures will go some way to providing the means to provide a safe environment for care where it becomes everyone’s everyday business.
