Manager Chronic Care Program (RN)
TheMontefioreMedical Group is recruiting for a full-time Manager Chronic Care Program to join a busy practicewhich provides over 30,000 visits annually.
Position Summary TheManager Chronic Care Program(CCRN) is responsible for developing, implementing andmonitoring the office-based Chronic Care Program. The CCRN will provide direction to the Chronic Care Program coordinators and office leadership team to ensure ongoing success of the Program. This position is responsible to uphold compliancewithMontefioreMedical Center’s policies/procedures, service excellence standards and regulatory requirements.
Qualifications • Current NYS Registered Nurse License/Registration • Bachelor’s Degree in Nursing • 2-3 years of direct management experience in ambulatory, hospital and/or health care setting
• Demonstrated skill in workingwith and building high performing teams.
