The pressure on NHS beds continues to attract numerous headlines. So-called “bed-blocking” is a situation where patients that are medically fit to leave hospital do not have the support they need to recover at home, while a patient that needs a hospital bed may remain in A&E until a bed becomes available.

This is why Hilton developed the hospital discharge service. Led by nurses with the support of highly trained healthcare assistants, this service starts in hospital where some of the issues that delay discharges are unblocked. Staff then meet and greet patients on their return home. If required, they even provide the transport home. Once home, they address practical concerns including heating and food, and assess the patient’s need and the home environment and develop a plan to assist their recovery over the next five days. They also liaise with any ongoing care or support providers. Hilton recognises that a hospital stay can impact the patient’s confidence. It has a strong focus on helping patients regain confidence and reconnect with the community. Hilton believes that this greatly reduces the risk of re-admission. Hilton’s hospital discharge has been running for 10 months, and provides the service to three hospitals. Patients have liked the service, with 98% saying they would recommend the service.
