REGISTRATION CONFIRMATION Registration is NOT confirmed until payment has been received by MCCA. Electronic confirmation of your registration will be sent within ten business days of receipt of payment or upon completion of payment process if registering online. Full registration includes access to conference sessions, electronic course materials, and scheduled meals. Program sessions are subject to limited attendance due to facility capacity constraints, so please pre-register for your desired courses. MCCA is not responsible for any expenses associated with travel, lodging, or other costs incurred by the registrant, including any cost incurred due to cancellation of the conference. Audio or video recording or transcription of any portion of the seminar is prohibited unless express consent is granted by MCCA.

SUBSTITUTIONS AND CANCELLATIONS All cancellations must be received in writing; telephone cancellations will not be accepted. Cancellations received in writing by August 17, 2012, will be fully refunded minus a $50.00 administrative fee. Individuals canceling in writing after August 17 but prior to September 3 will receive a full credit towards attendance at MCCA’s 14th Annual Creating Pathways to Diversity Conference (only monies paid for the 13th Annual Creating Pathways to Diversity Conference will be credited; after applying credit, any balance due is the responsibility of the registrant). If written confirmation of cancellation is not received by September 3, no refunds or credits of any kind will be issued. Credits will be processed after the meeting. Substitutions are always welcome; please submit a written request along with a completed registration form for the new individual in order to transfer the registration. Cancellations and requests for substitutions should be submitted to education@mcca.com.
