RECERTIFICATION INFORMATION If your certi fi cati on expires on Dec. 31,


2011, please make sure you submit your materials (online) on ti me. Directi ons on how to complete the online recerti fi cati on process can be found on the USAT website. Below are reminders for the top fi ve most common errors in recerti fi cati on submission: 1. Background Check: A new background


check with NCSI is required when you recerti fy — NO EXCEPTIONS. A red light determinati on means you will not be allowed to recerti fy. This applies to everyone, including anyone who has previously been approved by the ethics committ ee. (It is recommended to complete this step fi rst as it is typically the biggest delay in the approval process when done last since the checks take weeks to process someti mes by NCSI).


Note: If you get a message saying you have one on record and cannot complete a new one, you must contact NCSI directly to have the hold lift ed from your account. 2. CEUs: 5 USAT CEUs are required. The remainder can be USAT or non-USAT. 3. Documentati on: Documentati on is required for ALL CEUs. If you do not have any documentati on, you will NOT receive credit. 4. Online Form: Make sure you have completed the form in full before you submit. You CANNOT edit the form once you have submitt ed it or upload any additi onal materials. Any materials not uploaded/included on the recerti fi cati on form will NOT be accepted. 5. CEU Value: The recerti fi cati on page has many USAT and non-USAT CEU examples listed. Keep in mind that a recerti fi cati on of another certi fi cati on, such as USAC, NSCA, USAT RD, is worth half of the original certi fi cati on value. Example: USAC is listed as 10 non-USAT CEUs for an original certi fi cati on received. A recerti fi cati on of your USAC certi fi cati on is therefore worth 5 non- USAT CEUs.
