Alveolar Ridge Preservation: A Novel Approach

Preserving alveolar bone following an extraction has become a gold standard in implantology. In this dynamic presentation, steps leading to facilitating prosthetic based surgical implant placement will be reviewed, including atraumatic tooth extraction, wound healing sequence of extraction sockets, and the rationale for alveolar bone preservation. Many techniques offer excellent results, but are surgically demanding and not necessarily cost effective to our patients. A novel, simple, and predictable method of ridge preservation will be discussed.

This seminar will highlight the increasing problems facing dental clinicians in the era of Implantology. Implant complications, failures, and peri-implantitis are major clinical issues impacting on patient care and treatment planning. Local and systemic etiologies are identified for peri-implantitis and selected implant failures. Clinical approaches for implant rescue continue to be evaluated and will be presented, including applications of lasers. Objectives: 1. Define peri- implant mucositis and peri-implantitis; 2. Recognize the primary etiologies of peri-implantitis; 3. Treatment plan non- surgical approaches for implant rescue; 4. Treatment plan surgical and laser approaches for implant rescue; and 5. Recognize the importance of implant maintenance.

On Marginal Bone Loss Around Oral Implants Tomas Albrektsson, MD, PhD, ODhc


Some scientific papers have claimed very high frequencies in the range of 6-50% of all implants allegedly suffering from peri-implantitis with substantial bone resorption. In addition, so-called ligature studies have been presented in animal research without realizing that ligatures are but foreign bodies that, if combined with a titanium implant that is likewise a foreign body, gives rise to dramatic marginal bone resorption. When analyzed in critical studies, clinical results are significantly better than alluded to in these different studies; in reality about 1-2% only of controlled oral implants placed by properly trained individuals display dangerous bone resorption. Attempts to differentiate between different implants with respect to the level of marginal bone resorption, as performed in a recent study by Derks and co-workers, are questionable due to very small samples and statistical analyses that may be criticized. Osseointegration represents a foreign body equilibrium that will present long- term oral implant function, if not disturbed by factors that interrupt the equilibrium. Peri-implantitis is an immunological adverse reaction resulting in marginal bone loss which may or may not be complicated by bacteria that also may lead to increased bone loss.

Optimizing alveolar ridge volume and tissue quality is an important part of modern implant therapy. Regeneration of lost alveolar ridge anatomy can be accomplished with a variety of material combinations and techniques. However, selection of scaffolds and membranes may influence treatment outcomes. This program will describe the basic principles of implant site development, with an emphasis on GBR, including pertinent clinical cases. Participants will also learn how to select the right scaffolds and membranes to achieve the expected outcomes in specific sites and indications.
