He currently is the only surgical oncologist in Spokane, and one of only a half dozen in the western U.S., who performs Hyperthermic Intraperitoneal Chemotherapy (HIPEC) surgery. A “last chance” for patients who have cancers that have


spread to the


peritoneal (abdominal) cavity, and do not respond to traditional chemotherapy and/ or surgery, HIPEC surgery involves the removal of every bit of visible cancer in the abdominal cavity, after which the cavity is filled with hot liquid chemotherapy for 90 minutes, bathing all of the internal organs in direct contact with the cancer-fighting drugs. Only a hundred or so doctors in the U.S. perform the surgery. “There’s a long learning curve,” says Holbrook, who has been performing the surgery since 1996, and averages 30 to 35 HIPEC surgeries per year. “It takes a certain mindset and skill set to go into the abdominal cavity that is often filled with cancer.” The surgery is grueling on both patient and surgeon, lasting anywhere from 7 to 12 hours. For the patient, there is then a three to six month recovery period, and it isn’t without risk. “The complication rates are 30 to 60 percent,” says Holbrook. Despite these drawbacks, Holbrook is enthusiastic about the surgery and the hope it offers patients. “Some of these patients have a 75 percent or better chance of long-term survivorship. It’s not effective for all cancers in the abdominal cavity, but is best suited for appendix and colon cancers. It is used for other diagnoses as well, including patients with mesothelioma in the abdominal cavity and some ovarian and stomach cancer patients.” “For some of the cancers where we would think that there is no hope for them, there is hope,” says Holbrook.
