Proposals must be submitted on proposal forms. Proposal forms are to be placed in the special envelope furnished by the District and shall conform to the terms and conditions of the Bidding Requirements attached thereto. Bidders are to include with their proposal signed copies of any addenda, or acknowl- edge receipt of any addenda, if the District issued any addenda to this contract. Failure to do so may be cause for the rejection of any bid.


Each proposal must be accompanied by a bid deposit in the form of cash (U.S. currency only), cashier’s check, certified check or bid bond, as stated in the Bidding Requirements and Instructions to Bidders contained in the contract document. The bid bond must be underwritten by a surety licensed to do busi- ness in the State of Illinois, listed in the latest copy of the Federal Register and approved by the Director of Procurement and Materials Management.

On January 25, 2012, the Division served a Notice of its intention to enter a Order approving said application, pursuant to Section 60 (620 ILCS 5/60) of the Illinois Aeronautics Act. No comments have been received by the Division on this matter.


IT IS, THEREFORE, ORDERED BY THE DIVISION OF AERONAUTICS AS FOLLOWS:


1. No operations from said private-use vertiport are authorized by this Order until the Certificate of Approval has been issued hereunder.


2. Unless the Division grants an extension of time for completion, this Order shall become null and void unless construction is satisfactorily completed within 18 months from the effective date hereof. If all requirements of the Division are met upon completion and final inspection, a Certificate of Approval for said private-use vertiport shall be issued to AFCO Nighthawk Vertiport, LLC. Applicant shall notify the Division within 30 days after the completion of construction.


3. Said private-use vertiport shall conform to all requirements (including written notice of abandonment) of this Division.


4. Unless the Department finds that a hearing is necessary or that a longer period is appropriate, this Order shall take effect twenty (20) days after the service date thereof.


5. This Order and Certificate of Approval create no private interest in the applicant or its successors. The Division expressly reserves the authority to revoke or modify said Certificate when in the public interest to do so.


6. This Order and any Certificate of Approval issued pursuant thereto do not pre-empt, waive, suspend or modify any zoning regulations, building code requirements, or local ordinance adopted by local governmental bodies.


7. The Certificate of Approval must be modified or reissued if any alteration or extension is made to the private-use vertiport.

PARCEL 1: Transportation


Authority (“RTA”) invites you to sub- mit a proposal to provide graphic design services for RTA 2011 Popular Annual Financial Report. The contract to perform this service will be awarded on the basis of com- petitive proposals.


The RTA has established a Disadvantaged Business Enterprise (DBE) Program in accordance with regulations of the U.S. Department of Transportation, that wherein sub- contracting possibilities exist, the DBE subcontracting participation goal for this contract shall be at least 19% of the total contract amount. Proposers are to review and fulfill all requirements set forth in the RFP, including the Affirmative Action requirements. Failure to follow instructions set forth in the RFP will render proposals non-responsive.


A pre-proposal conference will be held on March 12, 2012 at 10:00 a.m. in the RTA Board Room, at which time questions regarding this Request for Proposal will be answered. Questions regarding this Request for Proposal will not be accepted or answered after this pre- proposal conference.


One original and five (5) copies or one (1) original and one (1) CD or DVD of your proposal must be sub- mitted to the RTA no later than 4:00 p.m. on March 19, 2012. Proposals submitted after the designated time and date will be rejected.

Residents, public officials, local businesses, school board members, college administrators, faith-based organizations, community-based organiza- tions, foundations, etc. are encouraged to attend the meetings to support the process and learn more about the plans to revitalize the community. Residents of the Grand Boulevard Community and their children ages 16-21 are espe- cially encouraged to attend the meetings to learn more about the Choice Neighborhoods process and participate in the development the new communi- ty.
