Athletes wishing to participate in Special Olympics programs must have a current Special Olympics medical form on fi le at NWSRA and Special Olympics Illinois (valid for two years from the date of the examination). Please contact Glenn Anastacio to check expiration date or request a form. A medical form is available on our website,
www.nwsra.org. Original doctor's signature required, therefore faxed forms will not be accepted.
