Center School Receives Lump Sum Grant from Windham PTA

Pelham Healthcare Associates Receives Patient-Centered Medical Home Distinction


Pelham Healthcare Associates (PHA) is pleased to announce that it has been awarded with recognition as a Patient-Centered Medical Home by the National Committee for Quality Assurance (NCQA). This is a prestigious distinction awarded to only a few free-standing health centers in New Hampshire, and Pelham Healthcare Associates scored level 3 status, the highest recognition obtainable. This recognition is in addition to the one PHA received last year from NCQA for quality diabetes care. A Patient Centered Medical Home (PCMH) is


an innovative model for improvement of primary care. Each patient has an ongoing relationship with a personal physician who leads an organization that takes a collective responsibility for patient care. A physician-led team provides for patient healthcare needs and arranges for appropriate care with specialists. In addition, recognition of a practice requires evidence of ongoing improvements and dedication to the delivery of healthcare of the highest quality. To receive recognition, Pelham Healthcare Associates met NCQA-required standards in the following areas: use of an electronic medical record,


practice-wide measurement of quality, use of patient tracking registries, structured care plans for patients with chronic illnesses, educational resources in multiple languages, processes to implement improvements, and screening, care, and follow-up for high-risk patients. The recognition by NCQA underscores the high commitment to patient care that Pelham Healthcare Associates has had as their mission since the inception of the practice. We value our physician-patient relationship and provide high- quality healthcare to all of our patients. We strive to continue to make improvements in our practice and in our service to the community. Girl Scouts Work to Earn Money Matters Badge


Salem High School’s Matthew Cannone was recently awarded the John E. Burke Sportsmanship Award. This award is presented annually to a high school senior boy and girl who exemplify the principles of sportsmanship, integrity, and dependability on and off the basketball court. Candidates for the award are nominated during the season by IAABO basketball officials throughout the state. Nominations are validated by coaches and others knowledgeable about the background of the nominees. The award recipients are selected by a vote of the members of the state executive committee. The award takes on added prestige when you realize that this is one of the few awards where the class or size of the school does not play a part in the selection process. There are only two award winners for all of New Hampshire. The award is sponsored by the NH State Board of Approved Basketball Officials in honor of the late John E. Burke, a former president of our board who officiated high school and college


games for 33 years. He was one of the top officials in New England. The ability of Mr. Burke to set the highest standard of conduct during games was an inspiration to thousands of players and officials during his career. After Mr. Burke passed away in 1962, the New Hampshire Basketball officials chose to remember him and continue his ideals with the establishment of the John E. Burke Sportsmanship Award. Each award recipient receives an individual plaque to commemorate the award. The permanent plaque will be displayed at Salem High School for the next year, until another recipient is chosen.

girls work towards earning their Money Matters Badge. The field trip included a tour of the club, which allowed the troop to gain a new perspective of the club’s daily functioning, as well as the value of a dollar, learning money-saving tips for shopping on a budget. “Hosting these tours and activities brings such a warmth and joy to our team members,” said Danielle
