Monoclonal Gammopathy


Serum Protein Electrophoresis Serum Immunofixation


Serum Free Light Chain Assays (Free k, Free l and k/l ratio)


Add 24-h urine immunofixation if suspect AL amyloidosis


Abnormal Normal


Monoclonal band and/or


abnormal k/l ratio (Normal ratio: 0.26-1.65)


Perform


24-h urine protein electrophoresis and


immunofixation and continue work-up


<1% of monoclonal gammopathies.


Continue work-up (e.g. bone marrow biopsy) only with high clinical suspicion


An algorithm including Freelite will allow for the most sensitive and specific identification of all significant monoclonal proteins and negate the requirement for urine samples for screening for Bence Jones Protein, unless there is a strong clinical suspicion of AL amyloidosis.
