Treatment Timeline Framework


Pre-admission goals include: • Assessment report completed detailing recommended level of security


• Detailed list of mental health and learning disability needs and risks to be met in the first 24 hours/week; multidisciplinary input (including speech and language therapy); reasons admission is necessary and what outcomes will be aimed for; Responsible Clinician allocated


• Recommended visit to the service and/or information pack about unit sent out (beginning of Patient Portfolio) DAY ONE/ADMISSION DAY


The Primary Nurse, Associate Nurse, Care Team and Key Worker are allocated prior to admission. The Primary Nurse (or nominated deputy) introduces the patient to the service and completes initial admission documentation, which includes:


• Read patient their rights (as required under Section 132) and record their level of understanding in their notes. This will be repeated at monthly intervals thereafter, unless it is required more frequently. At this point the patient will also be made aware of their right to access an advocate, as outlined in the Mental Health Act Code of Practice.


• Commence the holistic nursing assessment and care plans • Record baseline physical observations • Confirm spiritual, cultural, dietary and physical health needs • Implement risk prevention and management plans (including personal security risk plan) • Public protection arrangement arranged (if needed)


Admission by Medical Practitioner/Responsible Clinician, which includes: • Initial mental state and physical examination • Screening to include communication and dysphagia assessment • Confirm observation levels and required medication • Initial formulation of need and risk assessment • Clear statement of why the patient requires this treatment/placement


PiC Learning Disability Secure Services


This is a broad timeline framework for patients with a learning disability who are admitted to a PiC hospital, following the pre-admission process and related multidisciplinary team (MDT) meeting. The assessment and treatment of a patient is steered by their care and treatment plan; components of the package will be adapted to meet individual patient needs.
